The Health Care Center at The Peaks is the premier long term care and rehabilitation facility in Northern Arizona. The Health Care Center at The Peaks is certified by Medicare and the Arizona Health Care Cost Containment System (the Arizona Medicaid Program) and contracts with most long term care and many acute care health plans.
The Health Care Center is staffed around the clock with Registered Nurses, Licensed Practical Nurses and Certified Nursing Assistants and has a fully staffed rehabilitation team consisting of physical, occupational, and speech therapists. We are very proud of our reputation for returning patients to the community. Our average length of stay for rehabilitation patients is just over 21 days.
